Introduction: Chylothorax occurs upon rupture, tear or obstruction of the thoracic duct or its major tributaries, resulting in the release of chyle into the pleural space. It occurs most frequently associated with trauma or malignant lesions; but other causes have been described.
Development: The development addresses everything related to spontaneous chylothorax. In addition, the diagnosis, treatment and different clinical cases where the subject is approached are discussed. Reaching the conclusion. with the future lines of investigation with respect to the thematic approached in the article.
Conclusions: The management of chylothorax is still controversial, with variable results. It is predominantly conservative: ventilatory support, drainage, total parenteral nutrition and feeding with medium chain triglycerides. If conservative treatment fails, surgery is usually required, which is not without risk.
