Prognostic surgical factors of neuroblastoma at the Oriental Regional Center of Pediatric Oncology in Holguín, Cuba

Abstract

Introduction: Neuroblastoma is the most common solid neoplasm outside the cranial cavity in children. Its particularity is reflected in several clinical and biological characteristics which are considered to predict therapeutic outcomes.

Objective: To identify the prognostic surgical factors in patients diagnosed at the Oriental Pediatric Oncology Center in Holguín, Cuba, during the period from January 2010 to December 2022.

Methods: A retrospective descriptive study was carried out in 18 patients in whom described the variables: epidemiological clinical, imaging methods, pathological diagnosis, type of surgery and evolution.

Results: 50 % were diagnosed before one year of life, predominantly in girls (66.65 %), in the abdominal location and with involvement of the left adrenal gland (87 %). Ultrasonography, in association with computerized axial tomography, allowed the initial diagnosis in 94.50 %, and it was very useful in the identification of surgical risk factors (78 %). The pathological diagnosis showed a predominance of undifferentiated grades in 66.70 % and in more advanced stages III and IV (55.60 %). Primary (28 %) and secondary (44 %) surgery was performed, with 28.0 % deaths.

Conclusions: This disease continues to be a surgical challenge and, in the absence of molecular and cytogenetic determinants, clinical and histopathological factors are those of special relevance and prognostic correlation. The standardized transition towards the use of the pre-treatment classification of surgical risk by imaging is fundamental for the definition of the therapeutic modality to be used.
