Right testicular tumor in a lactating patient treated at the Esmeraldas hospital, Ecuador; case report and bibliographic review

Abstract

Introduction: Testicular neoplasms, although they only constitute 1% of male solid tumors, are the most common malignant solid cancer in men. They are divided into germ cell tumors (GCT), which cover 95% of cases, and sex cord stromal tumors (SCST). Histologic evaluation and staging are essential to determine treatment, which may include orchiectomy, retroperitoneal lymph node dissection, or chemotherapy.

Objective: To present a case of right testicular tumor in a pediatric patient, an unusual presentation.

Case presentation: A 7-month-old infant was diagnosed with a right testicular tumor and underwent a right radical orchiectomy and a left orchidopexy. The post-surgical evolution was favorable, and the patient continued with follow-up in the outpatient clinic.

Conclusions: The management of a testicular mass in pediatrics begins with scrotal ultrasound and evaluation of tumor markers to determine malignancy and plan surgery. If the mass is benign, surgery is performed; If it is malignant, radical orchiectomy is chosen to avoid dissemination. Localized tumors are monitored with imaging and markers, while metastatic tumors require chemotherapy.
