Immune reconstitution inflammatory syndrome in HIV patients

Introduction: Immune reconstitution inflammatory syndrome is a clinical syndrome that has been described in patients infected with human immunodeficiency virus after starting the antiretroviral therapy because of viral suppression and an increase in the number of measurable CD4+ T cells.
Development: It is an important complication after starting therapy, associated with considerable morbidity and mortality, especially in patients starting treatment who are severely immunosuppressed. Although there is considerable clinical and pathophysiological heterogeneity, features include: clinical deterioration in the first weeks or months of antiretroviral therapy, with evidence of tissue immunopathology and, occasionally, systemic inflammatory response.
Practical applications: The fact that it constitutes a predictable response allows to determine the initiation of therapy, whether at home or in a health institution, to plan the patient's follow-up and to predict that there is an occult opportunistic disease concomitant with HIV.
Conclusions: It is a frequent complication in HIV patients who start antiretroviral treatment, with a very negative impact due to the associated morbimortality and the health and social cost it implies. In the specialized literature there is little evidence on its behavior.
