Influence of the extraction of Bichat's balls on the function of facial muscles

Introduction: Bichat's ball removal, or bichectomy, is a popular cosmetic procedure used to refine and define facial features. This outpatient procedure is performed under local anesthesia using an intraoral approach that avoids visible scarring. Bichat's Balls are fat masses in the cheeks that facilitate intermuscular mobility and protect neurovascular structures.
Objective: This article reviews the anatomy and function of Bichat’s balls, removal procedures, and the aesthetic and functional impact of bichectomy, as well as associated risks and complications.
Case presentation: Bichectomy is performed under local anesthesia using a small intraoral incision to remove the fat balls. Immediate results include a slimmer face and more defined features, which are visible almost immediately and consolidate over several months. However, complications such as hematoma, infection, asymmetry, and long-term effects such as accelerated facial aging due to loss of fat volume may occur.
Conclusions: Bichectomy offers immediate aesthetic benefits but may accelerate long-term facial aging. Complications include hematoma, infection, and asymmetry, which are manageable with postoperative follow-up. Extraction impairs muscle mobility and neurovascular protection, affecting facial aesthetics and functionality.
