Perforation of Meckel's diverticulum due to fishbone; an unusual presentation of acute abdomen in adults

Introduction: Meckel diverticulum (MD) is the most common congenital anomaly of the gastrointestinal tract, it is usually asymptomatic, and can present with obstruction, hemorrhage, inflammation and perforation. Perforation due to foreign bodies is considered a complication with a low incidence and at the same time even rarer than this being due to a fish bone that can cause symptoms of an acute abdomen. The pathophysiological mechanism consists of the accidental ingestion of fish bone, passing and entering the lumen of the diverticulum and subsequently intestinal peristalsis mobilizes it until it perforates the full thickness of its wall.
Objective: To publicize this rare presentation of Meckel perforation by a fish bone.
Case presentation: 25-year-old male, who presented with an acute abdomen, with very intense pain in the right lower quadrant and who during surgery identified a Meckel's perforation due to a fish bone 70 cm from the ileocecal valve.
Conclusions: A complicated DM should be taken into account in the evaluation of patients with acute abdomen in the context of right iliac fossa pain syndrome. And in cases of perforation, regardless of the cause, surgical resection is mandatory.
