Perforation of the cecal appendix inside the abdominal wall manifesting as a wall abscess, an unusual presentation

Introduction: Appendicitis is the most common abdominal surgical emergency in the world, with a risk of 8.6% in men and 6.9% in women. It is the most common abdominal condition in hospital settings. However, complications such as perforation with abscess formation in the abdominal wall with consequent soft tissue infection are rare events nowadays. Analyzing that soft tissue infections are very common in adult patients, and generally present with redness, swelling and pain. Late diagnosis contributes to the appearance of complications. Emphasizing that, due to the progression of the pathophysiology of the disease, sometimes the appendicular tip can perforate and produce a fistulous tract towards the abdominal wall, discharging its contents, producing an abscess in the abdominal wall.
Objective: To present this rare complication of acute appendicitis, clinically manifested with an abdominal wall abscess.
Case presentation: 58-year-old female, with acute abdomen plus soft tissue sepsis due to extension of the appendiceal disease towards the abdominal wall.
Conclusions: An intra-abdominal origin should be considered and tried to rule out in the differential of soft tissue infection of the abdominal wall.
