Body Mass Index: Precision for the Diagnosis and Management of Obesity

Summary Obesity is the excessive accumulation of adipose tissue due to an imbalance between calorie intake and expenditure, emerging as a significant global health problem associated with cardiometabolic disorders that increase population morbidity and mortality.
The Body Mass Index (BMI) is employed to assess nutritional status, with its origins dating back to Adolphe Quetelet in the 19th century; however, its widespread use began in 1972 with Ancel Keys, who validated the weight/height2 formula. The World Health Organization (WHO) endorsed this by defining cutoff points (overweight ≥ 25, obesity ≥ 30), which have been questioned by studies suggesting variations across populations.
Research in various regions has proposed alternative cutoff points based on specific population characteristics. Studies suggest that BMI may not be a precise indicator of obesity in all populations. This stems from arbitrary cutoff points and its inability to consider lean mass and body fat distribution. It is argued that, as a sole indicator, BMI may misclassify individuals, leading to delays in timely diagnosis and treatment.
There is a need to establish specific cutoff points for each population, emphasizing the importance of conducting specific studies that consider ethnic, racial, and geographical diversity.
